DESCRIPTION:
Family Youth Interaction Community Services is currently seeking a Collaborative Services
Coordinator. Under the guidance and supervision of the FYI Management Team and Senior
Coordinator, provides coordination related services to children, youth and families; and performs
other duties as required. Qualified candidate must be able to demonstrate the following:
Demonstrates respect for children and families, ensuring involvement in all aspects of planning and
support activities. Utilize a strength based approach. Conduct all activities in a culturally sensitive manner adhering to recognized ethical standards.
Demonstrates a comprehensive knowledge of children and family human services system and community resources. Promotes positive organization and team relationships that support a collaborative, community-based support system.
Demonstrates the ability to facilitate meetings and work well with a divergent group of people; accurately listen; develop trust of team members; understand multiple perspectives; effectively manage conflict; accept feedback without reacting defensively; provide support and encouragement; and maintain and demonstrate patience.
Completes all paperwork accurately within established timelines.
Pursues growth and development through ongoing learning opportunities within the program budget. Maintains flexibility in taking on other responsibilities as needed by families, teams and management team.
Provides service coordination to assigned families which follow the nationally recognized wraparound principles and may include, but not limited to the following:

REQUIREMENTS:
-
Bachelor’s degree in Social Work or human services related field.
-
Minimum of two (2) years experience working with children, youth and families.
-
A combination of education and experience may be considered to fulfill position expectations.
-
Willingness to work as part of a collaborative team.
-
Experience facilitating teams is desirable.
-
Willing to successfully complete the CANS reliable rater training within three months of hire date.
-
Willing to successfully complete the Team training within three months of hire date.
-
Willing to successfully complete HIPAA and Mandated Reporter training within thirty days of hire date.
CERTIFICATES, LICENSES, REGISTRATIONS:
The incumbent of this position must have a valid Driver’s License and satisfactory driving record is required.
Must be able to pass background check and random drug screening.
